What is the correct way of heading up a letter to the editor?

If you are commenting on a previous letter in the paper is this deemed formal or informal?

Are the address details of both parties necessary at the top?

I assume 'Dear Editor' is suffice at the start and
'Yours faithfully' at the end?

Any other tips would be most appreciated.

In Br.E quality papers, it is usual to begin "Sir". Yes, put the addresses in, as you say - they won't print these. The style of your letter will depend upon the style of the newspaper and the letter you wish to respond to, but generally these are considered to be formal letters.
