Yes they are different. Putting accent to one side, there are differences in vocabulary, spelling and grammar. Of course we can understand each other and any differences in vocab can sometimes lead to a humorous situation, but they are different.

Yes, accent aside, it's hard to imagine not understanding a British English speaker. (Some of those folks from the Scottish highlands can be a bit tough for me to make out, accent-wise, and I expect a deep Cajun accent would be hard for a Brit to make out.) Someone who didn't realize she was reading a British text might think the spelling a little weird and there are some prepositions "missing" to our ears. But the figures of speech might catch us by surprise.

Only if you are an actor or you are messing about with an accent for a joke or something - and I have to say that 95% of American attempts at British accents are appalling! I'm sure the reverse is true too. Most people can't even do authentic sounding imitations of the different accents of their own countries.
