How should I ask for an application?

Is it correct if I say... "Good morning, I would like to fill out an application" ?

Yes, and I would add the purpose of the application:


"Good morning, I would like to fill out an application for a new account."

"Good morning, I would like to fill out a job application."

[EDIT]  It does no harm to add "please", either.
