Bread and butter - single unit

Hi,

When do we treat two nouns separated by 'and' as a single unit? Can someone give me examples like the one below?

Bread and butter is my breakfast.

Essentially, any time the speaker (hopefully, with the assent of the listener) considers that it is a unit:

A song in your heart and a dollar in your pocket is the perfect cure for melancholia.
