I write a formal letter, and i need to give it to my teacher tomorrow.i would someone can check me this spelling and structure.

Dear Mr.  Roy ,

I am doing an English project on "heroin". ( full stop, not comma )   I want to know a little bit more about you and what happened when you were an addict.

I would like to ask you some questions about your past life as an addict.

The first question is "How were you exposed to heroin?"

I  would also like to know how frequent  you had been using heroin?

The second question is "How does an ex- addict overcome this condition?

But why you wrote this :"...how frequent  you had been using heroin?"
why not this : how frequent had you been using heroin?" - because it is question .

:"I  would also like to know how frequent  you had been using heroin" is an indirect question. (Please note that there is no question mark because it is an indirect question..)

"How frequent had you been using heroin?" is a direct question. (As it is a direct question, a questiion mark is required..)
