Comma question

Hello,

Quick question: I was writing a letter to my insurance company and I wanted to type all information in one sentence. However, I am having some trouble with the correct comma placement. Please read the sentence below and explain as to where I should place the comma, whether the commas I have already placed in the sentence are correct, whether the sentence makes sense with the commas, and what is the underlying grammer rule here.

sentence:

I, Tom Smith, would like to cancel my automobiler insurance policy, bearing policy number GH567565, with ABC company.

Please review.

I completely agree with Feebs.

It's often a mistake to try to put too much into one sentence. But in this case, you don't have to. They know who you are - you are signing the letter. They know who they are - you are writing to them.
