Do you put a "s" on "take?" and why? can I put an "ed" at the end of "dress"

He usually have to takes 10 seconds to get dress.

He usually has to take 10 seconds to get dressed.

You need to review the present tense of the verb 'have' - I have / you have / he, she, it has . . . 

'To take' is an infinitive form, so you don't put 's'.

'Dressed' is  apast participle. That's why you need '-ed'.

1.  The adverb 'usually' in your sentence indicates routine - therefore you need to use the PRESENT SIMPLE tense.  This particular tense needs the -s at the end for HE / SHE/ IT.
     That is why we say 'He usually takes...' 

THIS SENTENCE CANNOT BE SAID IN ANY OTHER WAY.

2.  get dressed -  You NEED TO put the -ed after dress because this is a situation in which the speaker is talking about something that he did / does himself.  In these situations we must use GET + PAST PARTICIPLEThe past participle ends in -ed when the verb is REGULAR (like to dress). 

Some other examples are:  GET MARRIED
                                        
GET DIVORCED
                                         GET WASHED

The above are all examples of regular verbs which take the -ed ending for the past participle.

Some examples of irregular past participles are:  GET LOST
                                                                       GET CAUGHT
                                                                       GET BROKEN 

The above 3 examples DO NOT end in -ed because they are NOT regular.
