Fall over/ down and lean over/ forward

Dear teachers
Can I tell me the differences between
A. fall over and fall down

fall down: to suddenly go down onto the ground after you have been standing, walking, or running, especially without intending to:
Ex:Lizzie fell down and hurt her knee.

 fall over:to fall onto the ground or to fall from an upright position
Ex:Tommy fell over and cut his knee badly.

I don't see any difference between them. Can I say, "Lizzie fell over and hurt her knee." and "Tommy fell down and cut his knee badly?"

B. lean over and lean forward

Ex. They were leaning forward, facing each other.  Can I say,"They were leaning over, facing each other?"
