Do we use a comma after 'but'?
Ex. But, he never hurt her.

It looks like a comma is not needed in this case; but its hard to tell without a context.

I'd rather avoid starting a sentence with "but."

But, he never hurt (hurts, not hurt) her.

For the above sentence, no comma is needed.

He hurt is not wrong; it's the past tense.

I believe the sentence is in the present tense.

He never hurts her.

He never loses his temper.

If it is in the past tense, it should be He didn't hurt her.

Certainly we need more information to understand whether it's a correct past or an incorrect present, but there's no reason why "never" can't be used in the past: Edgar Rice Burroughs, author of Tarzan, never visited Africa.
During their life together (now finished) he never hurt her.
I never visited an East European country under the old regime.
