Hi,

'The cream of the milk' refers to the best part of the milk.

'The cream of all jests (ie jokes)' refers to the best joke of all.

Old-fashioned accountants used to wear green visors, so that the light would not bother their eyes when they were looking at figures. Thus, 'green visor people' is a humorous way of referring to accountants.
