He had had .....

When I reading novels, I often find such sentence.

what's the difference between ..'He had' and 'he had had' ..'he had been'

He had is simple past-- a single past event:  He had a headache yesterday.
He had had and he had been are past perfect verb forms, relating to another past event:  He had had a headache and had been in a grouchy mood ever since the World Cup games started, preempting the MLB games.
