Can I say,

When Ali and John heard the incident, Lili took them to help the lady.

Yes, you can, but I'd tend to replace "incident" with a more specific word, such as "argument", "disturbance", etc., as appropriate.

One does not hear an incident, but:

When Ali and John heard the noise generated by the incident, Lili took them to help the lady.

 Or:

When Ali and John heard about the incident, Lili took them to help the lady.
