It means you trust your emotions (heart) over your intellect (mind).
You might love someone (heart) but you know that they are irresponsible and untrustworthy (mind). So do you listen to your heart or your mind?

Hi,
A similar, more common expression is 'his head rules his heart', or vice-versa.
