It's more often omitted in casual speech, and I'd say it's optional in formal writing.  But then, I'm not a guru.

How'd the boat hold up?  Have you put it in dry dock?

I've always thought that this contraction "How'd" means either "How would" or "had" (I'd been there etc.)

  I was reading something of his a couple of days ago, but it may have been old.

They've really screwed up the profile section so you can only see about a day and a half of posting history.  It lists a finite number of posts, which may go back in time, but the dates only go back 24 hours. It lists the thread names, so I guess you could go to the post and check the date.

Edit.  Okay, Optilang posted on Tuesday of this week.  http://www.englishforums.com/English/SimplifySentence/gqvvn/post.htm 

I did a site search for his name, and got to his profile from there.  His most recent activity listing (top of the list) was the one I've given the address for.
