When someone says 'how do you do' to me, do I have to say  'how do you do' again to him ? Can I say 'fine thanks'?

No, it's a rhetorical question - it's a highly formal greeting not an enquiry as to how you are.

You should always reply 'How do you do'.

But it's not used much these days - only in very formal situations.
