I wrote a poem to say thank you for a teacher. It's corny, but here it is:
It's the end of the year,
and I thought you should know
a part of me can't wait 'til Summer
and a part doesn't want to go.
It's not recess or lunchtime
or even time with my friends.
It's because I will miss you
that I don't want the year to end.
I put up other thank you teacher stuff on my website.

it depends on how you feel about her, and how she thinks of you as a student. you want her to know how amazing she is to you, and how you'll never forget her. be creative, but don't copy and paste from here, let her know how YOU feel. use fun words, but dont be too sappy. good luck!
