'sad'  instead of 'said'.

Tell isn't used idiomatically in the original example.

However, tell could be used with a different structure:

A told him/us that he was very sad.

In order to use the original structure, use say/mention:

A said/mentioned that he was very sad.

Hi,

We certainly say A said he was sad and B asked why

but for more careful English I'd prefer A said he was sad and B asked the reason.

Best wishes, Clive
