Is the point of the exercise to label the tense, or to write the the sentence using the tense indicated?

All of your sentences are grammatical. Few of them match the tense indicated.

A continuous will have an "-ing."

A past perfect will have a "had."

A present perfect will have a "have"

Your final sentence is in the passive.

I have struck out the incorrect ones:

a)       I haven’t spoken to him yet  - present perfect continuous
b)       Did you go to the pub last night?  - past perfect
c)      
They’re living in Paris now  - simple present continuous
d)       I didn’t know that I hadn’t locked the door  - past perfect
e)      
I’ve been thinking of leaving him for ages  - past perfect continuous
