Is the point of the exercise to label the tense, or to write the the sentence using the tense indicated?

All of your sentences are grammatical. Few of them match the tense indicated.

A continuous will have an "-ing."

A past perfect will have a "had."

A present perfect will have a "have"

Your final sentence is in the passive.
