Sub: Request for no pay leave


Dear Sir
I have been suffering some health problems for the last few months, so I am planning to go for a medical check up and treatment in India. Therefore kindly grant me 15 days no pay leave effective from 1st December 09 to 15th December 09.


I hope you understand my situation and will grant my request.
