Hi,

How about something like this?

Dear Mrs. Smith,

I would like to apply for medical leave, starting on 12th. January, because I have a fever.

Thank you very much.

Yours sincerely,

If you live in a country where people are very, very polite in letters, you might want to add some more polite words to this.

Best wishes, Clive

Dear Mr [Surname]

Because I have some personal engagements, I request you to grant me leave for the following days:  Monday, Tuesday and Wednesday ( March 24, 25  and 26).

Thank you very much for your consideration.

Sincerely yours,

(I suggest, however, that you will have a better chance of success if you give your boss an indication of what these 'personal engagements' are.)

Dear Ma'am:

Good afternoon!

I, Jocelyn a. romero, nurse II at rcb2 5th flr, would like to ask for an apology

for being absent from Feb. 20-March 31 because of family/medical problem.

I was very affected the day i got the news from my other sibling that my dearest sister was

in a very delicate condition in the intensive care unit where she rushed due

to pneumonia and diabetes complication.

Because of her unstable medical condition, i rather stayed at home to check

up her status from time to time to keep my sanity.

In lieu of the above situation, I am asking your deepest understanding why i

took a long leave.I know deep in your heart if you're in my shoes you will do the same.

Respectfully  Yours,

Jocelyn romero

Please kindly check on my grammar, punctuation mark and the reason for my leave !

Thanks!Happy
