I think you missed out commenting on the above words in bold.

I think 'Please' and 'kindly' cannot be used together. 'Please' alone is enough.

Thanks for the comment.
I left it as is because it seems that in many cultures a level of politeness is normal which seems excessive to me here in Canada.
A lot of these leave letters we get asked about would not even be written in Canada, or would be written in a very different way.
