I need to write a letter to the management seeking a months leave as doctor as advised me take bed rest during the treatment. I want to also infrorm them that I will be available on phone and internet. I will coordinate from home. (gyneacologist -test tube baby treatment)

Pls help me . i need a draft leave letter immediately.
