This all seems quite odd to me. Surely such matters have to involve personal contact, eg you have to go there in person, and give them a new sample signature? How will they know this letter is not written and signed by an impostor?
Should you not provide more specifics, eg your account number if you are writing to your bank? Why do they have your signature?
In addition, I have never heard of anyone forgetting their signature.
