Leave Application

I would like to apply for leave from 20 May through  6 July 2008.  This would be long term leave for my marriage.  My wedding date is 29 June.

Please give me a sample Marriage apllication. I've to write an apllication for Marriage leave to my Managing Director. But I don't know how to write. My marriage will be held on April 4, 2007. I want to take on week leave from 3 April to 9 April, 2007. Please help to write an apllication.
