Why it does not add a is there to make it as passive voice? From my understanding, passive voice comes in the form of
<object> <is/was/are/were/have been/has been etc> <past participle>
is this correct?

meant is a past participle used as an adjective here.

documents meant to stimulate ... skills, i.e., documents which were written with the intention of stimulating ... skills.

Not much different from these:

paper designed to catch flies:
This paper, though obviously designed to catch flies, has never been effective in cool weather.

a battery built to last five years
The battery, though built to last five years, stopped functioning after six months.
