Hi All,

I wrote the below letter but feel like it is too short and grammatically incorrect. I would appreciate if someone help me refine it. I have to apologize for not making up to the appointment and request for a new appointment and i need to give reasons for the cancellation of the appointment.

Thanks in advance.

The below is my letter
