What does 'never say die.' mean?
When do you use this phrase?
There is a song titled 'Never say die', which is the theme song of the movie 'Iron Eagle'.
Does the phrase have anything to do with the military?

It means "don't give up."

You can see how someone fighting a battle would want to encourage their comrades to not give up?

By the way, I also often hear 'Never say never.'.
Does this mean the same as 'Never say die.' or 'Don't give up.'?

Similar, but it's closer to "nothing's impossible".

It means that no matter how certain you think something is, sometimes things turn out differently or people may change.

IMO:

things may happen, although you said they would never happen

things may happen, which you said would never happen
