Hi,

1. Do not place a phrase or clause:
A. in a sentence. B. at the beginning of a sentence. C. at the end of a sentence. D. too far from the word it is meant to modify.
Answer D.
I don't like any of these answer. D is clearly the expected answer, but it seems a bit odd to me because I don't see that a phrase/clause has to 'modify' anything.

Best wishes, Clive
