I am writing to ask for your help to revise a request for recommendation letter. I want to ask a well-known professor who may hardly remember me to write a recommendation letter. I got a low grade for the course I passed with him. But I am obliged to ask him write a recommendation letter because the supervisor of the position I am applying has asked me specifically for his letter. I have the problem of how to manage to tell him what I need and let him remember me. Actually I was not as bad as my grade was!!! I referred him to people who I worked with and they have a positive attitude toward me. I prepared the following and I need your help to correct it:

Dear Prof XXX,

It has been a long time since I have had any communication with you and I wonder if you still remember me. I am Ahura Mazda and I passed the course of Borehole logging with you at the 2nd semester of 2002-2003 education year.

I am now applying for PhD positions at various universities. The position I am now applying for is XXXXXX supervised by Prof. YYY from ZZZ University. Please find the details at the following website:

I need your help with writing a recommendation letter to support me for this research position. During my short talk to Prof YYY, he asked me specifically about you and I am sure that your recommendation as a distinguished professor would be highly valuable.

Please find attached my CV for further details and recent activities. In case you could not remember me, please kindly refer to Prof AAA and Prof BBB.

Looks fine but your first sentence is a question so should end with a question mark - ?
