hi.........i wanted to write a letter to the chairman of a educational institute requesting him to send me a letter  coz my apllication form has not yet reached the office & i want to appear for the exam
this is how i would write it..plz suggest me changes if required

Dear Sirs,

   I wish to appear for the -----2009 examination, and I have filled out the application for it, but it has not yet reached your office and I have not received any acknowledgment.  I am enclosing a photocopy of my application form and the post office receipt with this letter.
