'Something' is is in apposition to that characteristic, ie to the phrase 'a bit lean and keen'. '(that) I know (that) Tony finds very unattractive' consists of two nested relative clauses describing 'something'. If you want to separate these sentences out a bit, you could rephrase this way. He is looking a bit lean and keen. I know that lean and keen is something that Tony finds very unattractive. Something that.... This is a relative pronoun. However, I know that.....that is a complementizer; that is, it is a noun clause, not a relative clause. A relative clause follows a noun, not a verb.
