Just wanted to know if this sentence is valid in a formal letter. (Correct Usage ?)

I hereby authorize Jim to collect the form on my behalf.

It sounds good but this is what I would say.

Jim is authorized to collect any forms on my behalf.

It's about two years since the question was posted, but thank you for your comment.  Your version sounds fine to me too: either would do, but I'd probably use the poster's original wording in a formal statement.
