3) The "complaint form" had not been completed as evidences were still being gathered.>>

It was verified that the auditee sent the revised dates of the project X to the quality department (responsible for updating the main projects list), who will be updating the main programme accordingly. >>

3) The complaint form had not been completed, as evidence was still being gathered. (No quotes necessary)

4) The programme for project X was included in the division’s main projects list but it was not updated. It was verified that the auditee sent the revised dates for project X to the quality [USUALLY CALLED QUALITY CONTROL, BUT CHECK THE SPECIFIC TERM USED BY THE CLIENT] department (responsible for updating the main projects list), who will be updating the main programme accordingly.

What would be the meaning of the sentence # 3 if the comma is missing before ",as evidence"?

It would probably be interpreted the same way, Rishonly, because then we would have the past perfect standing alone with no other past event, and with the uncompleted form and the evidence still being gathered.  Some logic is required, I think-- use commas wisely yourself, but never rely on them in others.
