Dear Madam,

 

This is to inform you that my family and I have been going through intense mental stress for a while, due to living separately in different countries. I believe that this is the right moment for me to apply for my husband and son to immigrate to Australia and to end this misery once for all.

 

For this reason, I would like to request for a leave of six weeks, which will include two weeks of annual leave and four weeks of unpaid leave. Would you be so kind in approving my leave, which will be from 01 March 2007 to 06 April 2007? I will be in Jakarta during this time and hopefully, I will be able to come back to Australia with my family before the end of my leave.

If at all possible, it will be better to have the name of the Nursing Manager and address her by name.
