told/urged her that she should go" The difference is in the verb. Part of the definition of the verb is that it takes "to" with the indirect object. (There's probably a more elegant rule.)
If you eliminate the indirect object, you may say,"Martin suggested [that] she [should] go.
Edit. I'm beginning to think I'm off base on this. I wonder if when you use "told/urged," her is actually the direct object rather than the indirect object.

I also notice my error with "urged." It should be "Martin urged her to go," or "Martin urged that she should go."
