 - All right. Can I get something to eat?
- It has to be to go.

What does the second person mean? Perhaps "You need to eat if you want to keep yourself alive"?

When you get food "to go" it means you take it out of the restaurant to eat later.
This person is saying that whatever they get to eat, they have to get it in carry-out containers, and not sit at a restaurant to eat it.

Barbara, who answers in American English. My housekeeping skills attest to the truth of the second law of thermodynamics: Left to themselves, things get more and more random!
