What does this mean?

Perhaps:

Keep your right palm up in front of you, with fingers tightly closed (closed fist).
Then open the thumb first. It indicates the right.
Then open the pointer next. It indicatest the left.
This is kind of a random choice ... / play ...

I'm with Clive. I've never heard this before. Surely I have control of how many fingers I hold up? Withoutu any more context, I can only say this doesn't make sense.

That doesn't make any sense to me either, you would know which way you would turn depending on which hand you used, there's nothing random there.

Was this line part of a joke? I suspect it may be more than a little rude after googling 'one finger, two finger', assuming that it isn't referring to the Hawaiin dish of that name. The other uses are all rather, um, anatomical.

Doesn't make sense to me either.  Grasping at straws, could he be counting out seconds, in a sort of "ready, steady... go" way, before randomly choosing a left turn?

What's the fuller context of this?

I think it is a joke and he is not really deciding which direction to go in as which woman (one at the end of each road) to go and see.
