Which one is more perfect?

1) How are you doing?
2) How do you do?
3) How you doing? [I've heard many people saying this way as well, in California and I'm not sure if the word "are" is silent]

and what about How you doing...well...I guess that it can be informal one. or even used in California but also as an informal version.
