Help: I need work experience certificate model.....

First, can you explain what a work experience certificate is? We don't use this term where I live.

Who writes it? Who receives it? What is it used for?

Where I live, we call this a reference letter. It would be signed by your boss. It would say where you worked, what your job title was, how long you worked there, and whether your work was satisfactory. Your boss would write it and sign it.

Where you live, does the employee usually write this, and then ask the boss to sign it?

I've made a couple of edits to the above.

Where I live, we wouldn't write this in this way, but other countries and cultures may find this suitable.
