How to write a f.l. if I need employer to send me p45?

I need to write a formal letter to get .a P45 from my last employer, but I have no idea how that kind of letter should look like...

I've been working for the company for a couple of weeks this summer, then I went back to my country, but didn't take p45 with me (they told me it will be sent to me within a week, but I moved from the place where I was staying in). Now I want to ask them to send me this document to Poland, because I need it if I want to get my tax back.
How to write it in a formal letter, so it would be understandable and official?

Can anyone help me?
