'Dennis realized that during the conference how inattentive he had been.'-- Not correct, but eliminate the 'that' and you'll have a correct sentence, meaning that he was still attending the conference when he noticed his earlier inattentiveness.  2. 'Dennis realized that during the conference he had been inattentive.'-- this is OK. Here he realizes the fact afterward.  3. 'Dennis realized how inattentive ha had been during the conference. '-- Also OK if you change the 'ha' to ' he '.
