1 What would be a good substitute for ailing in this sentence? -- they are not the only ones injured by cigarette smoke.
2 I don't think we can say, sicking or illing.Any grammatical explanation why they are incorrect? -- Yes. Ail is a verb; sick and ill are not.

Although smokers constitute the majority of people who suffer from cigarettes, they are not the only ones ailing from cigarette smoke.
