I think the examiner wants you to correct D to a reimbursement , or the more general reimbursement.  As regards the phrase &amp;#39;a hospital bill&amp;#39;, I think the examiner wants to talk about a bill that has not been previously mentioned, and thus does not want you to change &amp;#39;a&amp;#39; to &amp;#39;the&amp;#39;. In many of these types of test, you have to try to read the examiner&amp;#39;s mind a bit. According to the answer key, (D) is incorrect. I think reimbursement is a count noun and a general subject matter, so I would correct it to a reimbursement. Is that right? I also wonder why not use the hospital for (A) because I think it can refer to a specific item . Please advise.</description></item></channel></rss>
