 The Company ABC called me yesterday asking about my
information after I submit the job application a couple of months ago. After
the phone conversation, she (from HR department) emailed me a form asking me to
fill it out. She also asked about the copies of my certificates and others. 

 My question is how to send a very polite email to her.
