&lt;p&gt;The Company ABC called me yesterday asking about my
information after I submit the job application a couple of months ago. After
the phone conversation, she (from HR department) emailed me a form asking me to
fill it out. She also asked about the copies of my certificates and others. &lt;/p&gt;

&lt;p&gt;My question is how to send a very polite email to her.&lt;/p&gt;

&lt;p&gt;Here is my email to her to express my thankfulness.&lt;/p&gt;
