The expression 'to come to pass' means 'to happen' and isn't normally used in modern English. You'll find it mostly in the Bible and other old texts. The past tense would be 'came to pass', as you rightly assumed. You couldn't use 'come to pass' in the sense of 'arrive'.

Yes, ok. The verb "to know" is usually used in the simple past tense in some expressions with "never" or "always", for example. Other verbs are usually used in the present or past perfect with "never" or "always". I also see you are not quite sure whether to choose a present or past tense......

Thank you for your answer! Actually, I was confused by the form of the first, not the second verb. In other translations of this novel Past Tense is used with both "answer" and "say". Just can't see the reasons for grammatical expression of anteriority in the translation with Past Perfect....

In #1 it is an expression of will and in #2 it is a set phrase of hypothesis ('it is...time')-- both are examples of structures that take the past tense form (the subjunctive).
