Where have you been
I was in the hospital (first two lines are past tense. the person was in hospital but is not now.
Why are you there
Because I'm sick (these lines are present tense. the person is in hospital at the moment.
What is the matter
I'v an earache (I've, but I have an earache would be better.)
How are you feeling now
I'm good

You also need to put in question marks.

IMHO neither 3 nor 5 is correct. In these sentences 'every time' is redundant because 'should' implies 'every time'. The best one is 4. After 'should' the verb goes in infinitive (drink). However, the other verb (get) should have the present tense 's'.
By the way, the sentence is not a question, so the question mark shouldn't be there.
