i would to like to change my internet banking username and password due to login error for that i would like to request for a new one

so please help me out for writing a letter in a proper way

Start by drafting exactly what you would say if you were on the phone with them.

Remember to use proper English, like a capital I and words like "your" instead of non-words like "ur."
