This is the first time I have come across this, but I expect to start seeing it more in our business' correspondence with our clients. The letter that is on my desk for review contains the following sentence:


"Please include your child(ren)'s first and last name, date of birth, mobile phone number and email address, and return the form to us at your earliest convenience."


My only question is, what is the correct format for the single(plural) possessive that I have bolded? Thank you in advance for your assistance.

It's best to avoid the issue which can be done quite easily with a simple rewording:

"Please include for each child, their first and last name, date of birth, mobile phone number and email address, and return the form to us at your earliest convenience."

I agree with AS, but "your child(ren)'s first and last name ..." is also acceptable, as is "your child's/children's first and last name..."
