3) doctors recommended him that he did an angioplasty.

The first sentence is most understandable, the last two confusing!

The first one means the doctors told him that he should undergo the medical procedure angioplasty.

As another person responded, the first of the three sentences is the easiest to understand. However, it may be a bit more accurate to say "Doctors recommended that he have an angioplasty." or "Doctors recommended that he should have an angioplasty."
