It certainly was done in Toronto Canada in 1968 and 1969. You could drink alcohol in so called "Hotels" only. The hotels had usually 3 rooms, one for men only, one for women only and a room for mixed couples.
Salt was always on the table and I used it too. Do as Romans do.

You paid right away, but left all the change on the table for the next beer. That's how I remember it.
